护理病历读起来像AI填的表？7组实验：高级词汇没救，句子节奏才救

过渡词和高级词汇对护理叙事检测分数毫无帮助，句子节奏是唯一能同时拉低两个检测器的操作。

凌晨一点，你在sim lab里写med-surg的care plan，打着字突然发现自己正在打'Furthermore, it was determined that the patient would benefit from...'——你猛地意识到这不是护士在说话，这是表格在被填。所以我们做了个实验。两个独立AI检测器，七种改写策略，跑在真实的护理临床叙事上。数字全在下面。你先别急着看最后的'正确答案'，先看前两组，因为你会想把字典扔进利器盒里。

基线，未改写的AI护理叙事：检测器A读1.0，检测器B读0.9999。然后最经典的学生操作：撒过渡词。'Moreover, in addition, it should be noted that the patient...'检测器A：0.9999。检测器B：0.9999。你要么更糟，要么没变。过渡词在临床叙事里就是穿白大褂再打领带——你在表演正式，检测器秒抓你的表演。下一步：换高级词。'Utilized'替换'used'，'Facilitated'替换'helped'，'Demonstrated'替换'showed'。检测器A：0.9585。检测器B：0.9999。一个分数动了一位小数，另一个纹丝不动。护理写作里的高级词汇不是修复，是cosplay。病人不会因为你把'给了'写成'administered'就觉得自己被更好地照顾了。

现在看真正有意思的不对称。Burstiness，也就是混合句长，把检测器A拉到0.8867，检测器B拉到0.4443。B掉得猛，A几乎没动。然后wild card：第一人称加一个具体细节。不是一整段感官描写，就一个细节。'I noticed her left hand was shaking before I even asked her about pain.'检测器A：0.1958。检测器B：0.7885。A崩了，B几乎没动。这说明什么：两个检测器量的不是同一个东西。一个在听节奏变化，另一个在找锚定的人称视角。你没法用一个trick同时满足两个，你得叠。在临床叙事里，叠不是装饰，是'有人在房间里'和'模板被填了'的区别。

刻意不完美，意思是碎片句、一个hedge、一个拖尾的从句，给你0.58和0.4109。respectable。但赢家，那个我想让你截图的数字：刻意节奏。短句。然后一个长的，往上推。然后一个短的，砸下去。检测器A：0.2425。检测器B：0.1124。这是整个实验里唯一让两个检测器都大幅下移的策略，而且它不是词汇trick。在护理叙事里这不是文学把戏，这是你描述一个shift的方式。'BP 152/94. She was pale, diaphoretic, and her hands would not stop trembling. I paged the charge nurse before the next set was due.'三句话，三个beat，读者就在你旁边。你可以手练，也可以用easydue标出你节奏太均匀的地方，然后你自己决定哪句留、哪句拆。

easydue在这个实验里是什么角色？我们拿它最强的setting跑原始基线。检测器A：1.0到0.644。检测器B：0.9999到0.6349。它不是单项最佳，节奏在两个分数上都是更强的操作。但它是地板。它把机器默认的腔调拉到一个范围，读起来像一个疲惫的三年制护理学生在凌晨两点写med-surg轮转的记录。这才是你真正需要的register。Care plan不该读起来像期刊论文，该读起来像一个人站在房间里、注意到手在抖、打了那个电话。easydue不替你写叙事，它把多余的脚手架拆掉，让你的真实观察能落地。

三组before-and-after，从med-surg care plan里抽的，你preceptor凌晨三点会读的那种。❌ 'Furthermore, it was determined that the patient would benefit from the administration of oral analgesics, and it was recommended that the nurse monitor for potential adverse effects.' ✅ 'She asked for the oxycodone. Gave it at 2100, rechecked pain at 2200, she was asleep by 2330.' ❌ 'In addition, the patient demonstrated compliance with the prescribed dietary modifications, and the healthcare team observed a gradual improvement in nutritional status over the subsequent period.' ✅ 'She actually ate the chicken. Not the graham crackers. The chicken. By Thursday her albumin was creeping up.' ❌ 'The nurse provided emotional support, facilitating the expression of feelings related to the diagnosis, and encouraged therapeutic communication.' ✅ 'She did not talk much. I sat down, put my phone face-down, and said, You do not have to perform for me. She cried. I let her.'

检测器不是你敌人。它是你凌晨三点skimming chart的preceptor，心里想：这谁写的，教科书查房？做护士，别做字典，别做过渡词机器。这句话我建议你截图贴显示器上：'Care plan不是essay。是一个人站在房间里，对另一个人做了一个决定。就这么写。'节奏做工作，那一个具体细节做工作，你做工作。现在去改你的care plan，别让你preceptor读完觉得你抄了clinical skills handbook。
