你的交班记录写得像essay，护士长扫三秒就跳过了

交班记录不是essay。你越想写得专业，护士长越觉得你抓不住重点——临床写作的第一原则是信息密度，不是语言优雅。

凌晨四点，你熬完夜班，把交班记录往系统里一粘——三段话，全是"the patient was observed to…"it was noted that…语法无可挑剔。然后护士长扫了一眼，说"please summarize the changes"。翻译：你写了三句话讲了一件事，还让重点消失了。这不是语言水平的问题，是你把交班记录当essay写了。临床交班的第一原则是信息密度。不是语法，不是词汇量，不是"听起来像native speaker"。

SBAR框架 everyone knows it. But most international nursing students write it like a case report: full sentences, passive voice everywhere, every detail given equal weight. 这不是SBAR，这是论文摘要。正确的打开方式：S是一句话定位，B是背景压缩到两行以内，A只写你观察到的变化——不是写你"观察到了这个病人存在"。R是下一步，带时间、带药名、带剂量。没有形容词，没有过渡句，没有"in order to"。

来看真实对比。❌ The patient was observed to have a temperature of 38.2°C at 0600. It was noted that the patient was experiencing increased pain at the surgical site. The patient appeared uncomfortable. ✅ 0600: Temp 38.2°C, increased surgical site pain (self-reported 7/10, was 3/10 at 0200). Pain not relieved by PRN paracetamol. 看到了吗？右边没有"patient appeared uncomfortable"这种废话——数字就是uncomfortable，数字就是urgent。

❌ It is recommended that the patient should be monitored closely for any further deterioration. ✅ Watch for: fever >38.5°C, SpO2 <93%, wound dressing saturation. Escalate to Dr. Chen if any present. "Monitor closely"是空话。写具体的阈值。具体到能执行，才叫交班。❌ The patient's condition is currently stable and no further concerns were identified. ✅ No acute changes since 0200. Continue current plan. 最后一句——如果真没有变化，就写这句话，别写三段假装你观察了很多。

国际护理生最常见的错误：把"不犯错"当目标。你怕语法出错、怕用词不professional、怕显得不尊重病人——所以你堆了一堆缓冲词。The patient was reportedly experiencing… It was suggested that… The patient may potentially have… 交班记录里，不确定就写possible，别写may potentially。临床场景不需要hedging，需要的是你的判断。犹豫的措辞是交班时最大的安全隐患。

结构上，用bullet points。不是段落。不是完整句子。不是Furthermore, in addition to the above…。用缩写：NPO, PRN, IV, POD。用数字：时间戳、剂量、生命体征数值。用动词开头：administered, assessed, escalated, requested. 你的交班记录视觉效果应该是：一行一句，扫一眼就完事。读起来像交班记录不像论文，说明你做对了。

语言层面确实有坑——administer和give在交班里都可以用，别纠结哪个更正式；patient永远比the sick person专业；时态用过去式写已完成的事，用现在式写持续状态。这些细节丢给easydue过一遍，不用自己逐句检查。省下来的两分钟，用来确认阈值写对了、escalation路径写清楚了。交班记录里，结构错了会出人命，语法错了不会。
